Connecting Communities to Collaborative Care.

Collaborative community care aims to improve health outcomes, enhance patient satisfaction, and reduce practitioner and caretaker burnout by addressing the root causes of health issues as a team.

Every community should have access to comprehensive, individualized care within reach and this is the point of our collaboration.  We realize some communities lack resources and our services are designed to bring care to you and your community.

This model emphasizes collaboration and coordination among different professionals and clinics to provide comprehensive care. It involves interdisciplinary teams working together to develop personalized care plans tailored to the needs of the community.

Key components of our collaborative community care include:

  • Healthcare providers from different disciplines (such as physicians, nurses, naturopathic doctors, physiotherapist, kinesiologists, massage therapists, chiropractors, social workers, psychologists, psychotherapists, pharmacists, etc.) collaborate to provide care.

  • Involvement of community members and organizations in identifying needs, designing services, and delivering care.

  • Ensuring seamless communication and coordination among various providers and agencies involved in a patient's care to avoid fragmentation and gaps in services.

  • Focusing not only on treating illness but also on promoting wellness and preventing disease through education, screening, and lifestyle interventions.

Getting involved in showing care for your community can take many forms, depending on your interests, skills, and available time.

Remember that even small actions can make a meaningful difference in the lives of others and contribute to building a stronger, more caring community.

Here are several ways you can make a positive impact:

  • Start a community project focused on addressing a specific need or issue in your area.

  • Support local charities and non-profit organizations by participating in fundraising events such as charity runs, walks, or auctions. You can also organize your own fundraising event to raise awareness and funds for a cause you care about.

  • Offer your time and expertise to mentor students in your community. Many schools and youth organizations welcome volunteers who can provide guidance and support to young people.


  • If you are looking to get involved with some of our community clinics or other organizations please reach out to inquire about volunteer opportunities that align with your interests and skills.

  • Participate in local events that promote wellness in your community, whether that is care for the environment or care for others, all is welcome.

  • Offer assistance to elderly or disabled neighbours by helping with yard work, grocery shopping, or other tasks they may find challenging. Small gestures of kindness can have a big impact on those in need.
